Why is the nucleic acid of the whole staff endless?

This is the current situation in China: the operation of cities in the so-called "normal" places is actually maintained by uninterrupted 48-hour nucleic acid.

Early yesterday morning, Guangdong Province reported 5 new local confirmed cases and 5 local asymptomatic infections on April 8, including 2+1 in Guangzhou.

Presumably considering that this wave of Omicron virus is extremely concealed and the infection speed is extremely fast, the cities that have been successfully cleared before in China are all super-powerful means of "one case, the whole city will be closed and controlled". In the metropolis, all districts immediately carried out nucleic acid testing for all employees yesterday morning.

Nucleic acid of all staff in Guangzhou districts on April 9

Although the official and some big V have advised the public to "trust Guangzhou and don't need to stock up", as soon as the news came out, many supermarket shelves in Guangzhou were still sold out, because everyone knew that "the general trend of the world, if you seal it for a long time, you must check it. It must be sealed”, and what will it look like after it is sealed - just look at Shanghai and you will know.

Unconsciously, such a scene of "nucleic acid for all staff" has long become the new normal in our lives. Originally, Shanghai was the only one among the major cities in China that had never conducted nucleic acid tests for all staff, but this time it finally broke down. My community did the first round of nucleic acid in more than two years on March 20, and just did the seventh round yesterday.

This is still relatively small. The current round of the epidemic is far less severe than that of Quanzhou, which is in Shanghai. I heard that more than a dozen rounds of nucleic acid have been done in the past month. Since the Spring Festival this year, Wuhan has been reciprocating almost every three days, and has been tested more than 20 times. The epidemic situation in Zhengzhou at the end of last year was not too heavy in comparison, but there was a joke at that time, saying that in the future, if archaeologists find humans with cocoons in their throats, they must be from Zhengzhou.

This time, many people praised the success of the "Shenzhen Model". As for the reason, it was mostly due to its early closure, young population and strong execution. However, few people mentioned that from January this year, Shenzhen people have gone to any All places must show a 48-hour nucleic acid certificate. Although the lockdown was subsequently lifted, Shenzhen people still rely on 48-hour nucleic acid to go to work.

As of April 8, there were as many as 1,622 nucleic acid sampling points in Shenzhen, while Shanghai, with a population more than half its size, had only 201 nucleic acid sampling points on March 9, at the beginning of the outbreak. In other words, Shenzhen has already made sports a complete routine like a memorabilia. What is really surprising is not that it successfully closed the city, but that it has such an airtight prevention and control system, and it was still taken advantage of by Omicron. .

This is the current situation in China: the operation of cities in the so-called "normal" places is actually maintained by uninterrupted 48-hour nucleic acid. A friend joked: "As long as there is one positive case in a city, residents will enjoy the treatment of Yanjiao people entering Beijing when they go out; if there are eight or nine cases, they will enjoy the treatment of returning overseas people when they go out."

The first thing this full-staff nucleic acid model brings is an astronomical workload. To this end, Shandong recently developed a "nucleic acid detection robot", but even if it can greatly improve efficiency and save manpower, it will undoubtedly take time to promote and put it into use. The reaction to this news was more like watching a joke.

At least in the short term, a more direct way is to dispatch manpower from various places to help: According to the National Health and Medical Commission, as of April 6, a total of 38,000 medical personnel from 15 provinces across the country have rushed to Shanghai, 70% of which are invested in nucleic acids. In terms of testing - 4 provinces sent more than 23,000 medical personnel to undertake the task of nucleic acid sampling, and another 12 provinces sent nearly 4,000 laboratory nucleic acid testing teams, mobilizing a total of 2.38 million tubes/day of nucleic acid testing forces.

But this may still not be enough, so a third option is to employ lay volunteers in large numbers. For example, a group of people from each community should be trained now, similar to militia. This is actually the case in Shenzhen, Shanghai and other places. Although medical staff are still testing, community volunteers will check the health code, confirm identity information, open the nucleic acid code and scan it, so there are not many medical staff at each point. Because most of the preparation work is shared with volunteers.

Although the work has been decomposed, a large number of medical personnel are still needed. For most medical personnel, nucleic acid testing is an urgent task temporarily dispatched. It is a "part-time job" instead of a "job". Thinking of quickly cultivating a group of practitioners for nucleic acid testing? Through market-based recruitment, the cost may be lower and the effect is better. Some people joked on the Internet that the education and training industry should be transferred to do nucleic acid testing and sampling in the first place.

The above three methods, in the final analysis, are all filling the bottomless pit with resources, but when the resources obviously cannot keep up, the fourth method may be more important, which is to try to reduce the workload.

On April 1, the Changchun Municipal Health Commission announced that the next step would be to "scientifically organize and carry out regional nucleic acid testing to ensure that all tests should be carried out." Three days later, Jilin City in the same province followed suit. The nucleic acid testing of all staff is different, and this test will adopt differentiated testing methods for different groups of people.”

Residents of epidemic-free communities and villages announced by the Jilin City Epidemic Prevention and Control Leading Group, as well as recovered and discharged patients with new coronary pneumonia will not participate in the test. Antigen testing will be used, and 5-in-1 mixed-tube testing will be used for community (village) residents who have cumulatively tested positive for 1-10 people in the community (village) since March 29. Residents of the community (village) who have not tested positive for the unit will be tested with a 10-in-1 mixed tube.

This is a noteworthy trend, because compared with the undifferentiated full-staff nucleic acid, it means acknowledging the limitation of one's own resources, and having to change the strategy, and use the stretched manpower to investigate key groups.

In fact, this change was made clear before. On March 22, the National Health and Medical Commission issued the "Notice on Printing and Distributing Regional Implementation Guidelines for New Coronavirus Nucleic Acid Detection Organizations (Third Edition)". Compared with the previous second edition, one of the main revisions is to emphasize "" Scientific and accurate", the previous "full staff" was changed to "area", and the size of the area was determined by the needs of epidemic prevention and control.

Epidemic prevention and disinfection in Changchun Station

Although the National Health and Health Commission has already stipulated, why did Changchun eat crabs first? A reasonable guess is: no money.

The population of Changchun is about half of that of Shenzhen, and its fiscal revenue is only one-tenth that of Shenzhen. Shenzhen can do nucleic acid tests in turns 48 hours a day. There are many free nucleic acid testing points in the city, but you must know that this cannot be really "free". All have a cost.

The epidemic broke out in Xi'an at the end of last year, and it was closed for 5 weeks. It is said that 39 million people in the province have undergone 400 million nucleic acid tests, 10 times per person. At the government purchase price of 5 yuan per person, it cost 2 billion yuan a month for this. It does not count the inestimable personnel cost and social cost.

The nucleic acid of the entire staff is just a surprise inspection, but it cannot guarantee that those who are not infected will not be infected the next day, so it has to be screened repeatedly, constantly "dynamically cleared", and isolate any suspected positive person. Come, but the tricky thing is that the process of full nucleic acid may instead create conditions for infection.

In many places, people were crowded when queuing up for nucleic acid testing, so many such incidents broke out in various places: residents who were originally quarantined in their own homes were infected during nucleic acid testing. Someone said mockingly: "All staff nucleic acid, Omicron's best gift."

Why carry out full-staff nucleic acid? Generally speaking, this is because the flow modulation is broken down.

The reason why Shanghai has not conducted nucleic acid testing for all staff in the past two years is that it relies on professional and efficient epidemiological investigation, because if the epidemiological investigation can find out the positives and close connections at the first time, and isolate them quickly, then other people really don’t need to. The detection is like a minimally invasive surgery, and the part of the lesion can be physically removed.

If you don’t have such an ability to adjust the flow, or, like the current Omicron epidemic, it has spread quickly and covertly when it is discovered, then the flow adjustment will be meaningless. At this time, if there is still a dynamic reset, we can only go to great lengths to find a needle in a haystack with comprehensive screening, and then conduct isolation. When the nucleic acid detection capacity is also stretched, we have to use antigen testing to assist.

On March 11, Zhang Wenhong explained why the antigen detection method was adopted, and the virus spread of the Omicron variant strain was greatly accelerated. There will be certain difficulties, and antigen testing is fast and inexpensive, and can be an effective supplement."

The problem now is that there is no reason why the virus should not continue to mutate, and new mutated strains spread even faster. The UK Health Security Agency (UKHSA) confirmed that the new strain XE, which was recombined from Omircon BA.1 and BA.2, spreads 9.8% faster than the current BA.2.

If we continue to follow up, it means that "the road is one foot high and the devil is one foot high" continues to increase, but there is no limit to the evolution of the virus, and our resource investment cannot be unlimited. Not only that, but our lives will forever be trapped in the endless cycle of nucleic acid for all members.

In fact, many people's minds are already trapped in it. A few days ago, a friend was suffering from the round after round of nucleic acid tests in Shanghai, but when I asked her, "Can you say Shanghai gives up nucleic acid testing?", she said in horror, "How can it work? How can you find out if you don't test for nucleic acid? "

In fact, Wuhan did not test nucleic acid at the beginning. It was not until May 31, 2020, long after the end of Wuhan's closure, that the whole city conducted nucleic acid tests for all employees. In a few days, it cost 900 million and tested nearly 9 million people. Only 300 asymptomatic infections were detected, and there was no case. Diagnosed.

It was the first time that a city in the country conducted a nationwide nucleic acid test, and Mudanjiang City, Heilongjiang Province followed suit, but when a reporter asked an expert if it would be extended to the whole country, Wu Zunyou, chief epidemiologist at the Chinese Center for Disease Control and Prevention, said at the time: " Whether large-scale screening and nucleic acid testing should be done by everyone, in fact, it is not necessary." He said that nucleic acid testing is mainly carried out in key areas, key positions, and key groups of people, and there is no need to screen everyone in communities without cases. check.

This is not just his opinion. At that time, Professor Feng Zijian, deputy director of the China Center for Disease Control and Prevention and an expert of the liaison group of the Joint Prevention and Control Mechanism of the State Council, also said: First, there is no need to conduct nationwide testing for all employees; second, considering the domestic testing capabilities , "Even if a nationwide test is carried out, all sources of infection or infected persons cannot be completely discovered."

The scene of Wuhan's lockdown

In less than two years, re-reading these words is like a lifetime. Many people say that the experience of Wuhan, don't let the people of Wuhan suffer in vain, but they turn around and forget how the people from Wuhan came here - how did they get tested positive if they didn't do all the nucleic acid tests at that time? How could the city be unblocked?

It's very simple. Those who stay at home will not be tested. Only the active crowd, medical care, insurance, volunteers, etc., will seek medical treatment after symptoms appear. Of course, the original virus strain in Wuhan at that time had a high lethality rate, a long incubation period, and relatively low infectivity, but it was also sealed at home, and the infectivity was basically negligible. Omicron had a short incubation period and a low lethality rate. cycle, just a shorter block time.

Since most people in this round of Shanghai epidemic can recover by themselves, and detoxification is also in the early stage of infection, most people will be fine if they get it, call an ambulance for those who develop severe disease, and test nucleic acid for the whole family. , should go to quarantine to quarantine. The same is true for other diseases. After seeing a doctor, the nucleic acid will be tested again. According to the results, the diagnosis will be made, and some resources will be reserved for emergency treatment.

Of course, many people would say that Wuhan was a helpless choice at the beginning: the fatality rate was high, and it never wanted to infect family members, but the nucleic acid detection capability was low, and confirmed cases could only be isolated on a large scale. If everyone now chooses to voluntarily isolate themselves at home after positive, they have to bear the possible risks, that is, they are not afraid of infecting their family members-the whole family is bound, are you really willing?

A Wuhan friend who had a painful experience during the 78 days of Wuhan's lockdown said that he would. Because compared with the suspicion, tearing, and abuse caused by the rejection of positive infected people among the family at that time, he said: "I have seen a lot, and I feel that like our family, the whole family is infected and quarantined together, it is not bad. Happy."

I would too. Of course, I also know that many things in the country are not what you want to do, and often you can't help yourself, but I believe that we need to change, and this requires everyone to change their own ideas. It's not like we can't do anything.
